General Power of Attorney for Care and Custody of Child or Children (Tennessee)
This Power of Attorney is a form, authorized by T.C.A. § 34-6-301 et seq., which provides for the appointment of an attorney-in-fact for the care of a child or children, including health care. This Power of Attorney form requires the signatures of the person giving another the power of attorney and the person receiving the power to be notarized.
