This is a statutory form that is provided in the South Carolina Code of Laws. It allows you to express your wishes and desires regarding whether or not your life is prolonged by extraordinary means or by artificial nutrition or hydration if your condition is determined to be terminal or if you are diagnosed as being in a state of permanent unconsciousness.
